MOVEZ DANCE – WAIVER, RELEASE & INDEMNITY AGREEMENT

READ CAREFULLY – BY SIGNING THIS DOCUMENT YOU GIVE UP CERTAIN LEGAL RIGHTS, INCLUDING THE RIGHT TO SUE.

1. Acknowledgment of Risks

I understand that participation in fitness/exercise classes involves physical activity that carries inherent risks, including but not limited to: muscle strains and sprains, joint injuries, falls, cardiovascular stress, and other physical injuries. I acknowledge these risks exist even when instruction and facilities are provided with care.

2. Release of Liability

In consideration of being permitted to participate in classes offered by Karen Zimmerman, I hereby waive, release and discharge her from any and all claims, demands, or causes of action arising from my participation in the fitness/exercise program.

3. Indemnity Agreement

I agree to indemnify and hold harmless Karen Zimmerman from any and all claims, demands, or actions brought against her, including legal costs, that arise out of or are connected with my participation in the fitness/exercise program.

4. Assumption of Risk

I voluntarily assume all risks associated with participation in the fitness/exercise program. I confirm that I am physically capable of participating, and I agree to inform the instructor of any medical conditions, injuries, or limitations before each class.

5. Health Declaration

I confirm that I have consulted a physician if I have any medical conditions that may be affected by physical activity, or I accept full responsibility for participating without doing so.

6. Media Consent

I grant Karen Zimmerman permission to use my (or my child's) name, image, voice or likeness in any photographs, videos, or promotional materials, without compensation.

7. MINORS (UNDER 18 YEARS OLD)

If signing for a minor, I confirm that:

I am the parent or legal guardian of the participant.
I have explained the risks to the minor and they understand and accept them.
I consent to their participation and agree to be bound by this waiver on their behalf.

8. GOVERNING LAW

This Agreement shall be governed by and interpreted under the laws of the Province of Alberta. If any portion of this Agreement is found invalid, the remainder shall remain in full force and effect.

ACKNOWLEDGEMENT

I HAVE READ AND UNDERSTOOD THIS AGREEMENT. I UNDERSTAND THAT BY SIGNING IT, I AM GIVING UP LEGAL RIGHTS, INCLUDING THE RIGHT TO SUE. I SIGN IT FREELY AND VOLUNTARILY.
